Patient-Centered Medical Home

As current times are energized by innovation and technology, we are inviting patients, physicians and caregivers to re-imagine the healthcare environment of the future - a value based quality driven reimbursement system with preventative medicine at its core.

In 2008 in collaboration with four key medical professional societies – the American Academy of Pediatrics, the American College of Physicians, the American Osteopathic Association and the American Academy of Family Physicians, The National Committee for Quality Assurance (NCQA) first developed the “Patient-Centered Medical Home” (PCMH) program.

The NCQA PCMH program is a team-based, physician-led model of health care delivery that emphasizes care coordination and communication.  The aim of this model is to transform primary care into "what patients want it to be." It replaces episodic care based on illness and patient complaints with coordinated comprehensive long-term primary care – from birth throughout life. The healthcare team works together to provide the best possible care making sure the patient gets the care needed - in both wellness and illness- to heal the mind, body, and spirit. The priority is on wellness and prevention. The PCMH transformation is a continuous JOURNEY rather than an arrival at a specific destination.

The NCQA awards “Recognition” to those practices that have undergone transformation and completed the rigorous criteria required. Today, the “Patient-Centered Medical Home” is driving some of the most important reforms in healthcare delivery and is the gold standard in primary care.
